To determine if your H2O MOP® is included in the Corrective Action you must first locate the Product Reference Number printed on a label on the back of your H2O MOP®. (PLEASE SEE THE SAMPLE LABELS BELOW)

Once you have located the Reference Label on the back of your H2O MOP®,

1. Please select the source where you purchased your H2O MOP® from the drop down menu.

2A. If your reference number starts with TDC or M-TDC, please enter the first five (5) numbers on the reference label that follow “TDC” or “M-TDC” in the box provided below.

2B. If your reference number starts with “M-H20M”, please enter the first four (4) numbers on the reference label that follow “M-H20M” in the box provided below.
